What kinds of personal information does Ultivate collect?

 

Teleservices 

If you use Ultivate's teleservices, Ultivate may collect name, age, gender, date of birth, contact details (such as an address, email address and phone number), information about symptoms you have experienced, or any existing disabilities. Only where relevant, Ultivate may also collect information about ethnic background etc.

For teleservices, Ultivate may ask for the phone number and location so that Ultivate can help direct emergency services to you if needed.

Online services and notifications

Ultivate may collect personal information if you transmit personal information to us through Ultivate's online services. When you use Ultivate's online services, Ultivate may use IP review location as well as other personal information for the purposes of identification and support.

If you choose to create a profile, Ultivate's Website may remember your information when you return to use the Website to provide you with curated information.

If you create a personal profile for Ultivate's online services, Ultivate may also collect additional information relevant to your circumstances and the services you are using. Ultivate will store this information you can access via Ultivate's personal profile using a password you create.

Ultivate will only record your email address if you send us an email, register to receive information or notifications from us, provide us with your email address when requested or create a personal profile.

You can unsubscribe from Ultivate's information and notifications by sending an email to info@ultivate.space at any time. You can also delete your personal profile at any time.

No attempt will be made to identify anonymous users (including users logging on using a pseudonym) or their browsing activities except where a law enforcement agency may exercise a warrant to inspect the Internet Service Provider's log files.

 

Personal information in Ultivate's work health functions

In the ordinary course of business, Ultivate may collect your personal information. This includes your name and business contact information, including your role title, work email address, office address, and work telephone and fax numbers. Ultivate primarily uses this information to communicate with you or the organisation you represent.

Ultivate usually collects personal information directly from you — for instance, when you use Ultivate's services or send us an email, letter or complete an online or paper form.

Telehealth and online services

With your informed consent, Ultivate uses and discloses personal information for the following purposes:

  • Assisting Commonwealth, State or Territory services  or departments of health to provide you with services  or to address issues you may raise with them;

  • Matching service delivery data with health information for service improvement;

  • Compilation and analysis of statistics relevant to public health and safety; and

  • Complying with its legal obligations.

With your informed consent, Ultivate may use audio recordings of all consultations (via telephone or video call) to help ensure that the advice it gives meets the highest standards of safety and quality and is equal to best international practice.

Ultivate may disclose your personal information to IT service providers in and overseas to provide the services. Ultivate uses IT, service providers, in Australia and the United States.

Ultivate also takes steps to ensure that Ultivate's records of your information records remain accurate, complete, and up to date, including by verifying the information with you each time you use Ultivate'sservices or from other sources.

 

Records relating to Ultivate's telehealth and online services are retained with health records legislation. After this period, if we no longer require the information for any purpose it was collected and is no longer required by law to be retained by us, Ultivate will destroy or de-identify it.

 

Your Role

Ultivate users also have an important role to play in keeping data secure. You are responsible for maintaining the confidentiality of your account details and password. Your passwords protect your personal information, and you are responsible for any activities that occur in your account or respect your use of this service. Please let Ultivate know immediately if you suspect that the security of your password or account has been compromised in any manner.

What about my clinical information?

Details gathered by your psychologist during an individual or group consultation are protected by strict confidentiality and privacy laws. Your health information is protected by Australian legislation, and Australian psychologists must adhere to these lawful regulations. Clinical details relating to your health are recorded via a separate portal that ensures compliance with the relevant legislative and regulatory requirements across the main markets in which Ultivate operates, including Australia / New Zealand, UK and Europe (GDPR), South Africa, USA,  Canada, Central and South America. 

 

What if I have a complaint?

If you have a privacy complaint or concern relating to how Ultivate has handled your personal information, please contact Ultivate as detailed below. Ultivate will investigate your complaint or concern and respond to you within 14 working days.

If you feel Ultivate have not adequately resolved your complaint or concern, you may contact the Australian Information Commissioner at www.oaic.gov.au.
